This was painful to put in; it felt bigger than a 18 gauge. It also wouldn't stay in my nose and the little ball on the inside end tugged at my hole. Wearing it caused my hole to become inflamed, and I've had my piercing for 6 months.
Aaesthetically, it was barely visible when I wore it, which was great.

Its very small and seems like it would be pretty unnoticable on your nose, assuming you can get it in. Bigger than 18 gauge.
